Early Alert Referral System for Faculty and Staff

The Early Alert Referral Form is part of LSSU's comprehensive Early Alert System, designed to identify students who may be at risk of academic difficulty or failure.

This form allows faculty (full- and part-time) and staff to alert the Office of Advising and Retention of any student who is not making satisfactory progress and/or is exhibiting behaviors that may lead to academic difficulty (i.e. non-attendance). Once a student is identified via the form below, the Office of Advising and Retention and/or appropriate support service personnel will attempt to contact and work with the student in an effort to improve the student’s academic performance and opportunity for success at LSSU. Simply complete the information below and submit online.

Before reporting a student for failing to make satisfactory progress in a course, it is strongly recommended that the instructor first discuss the situation with the student, if possible.
