Form Instructions

  • The University's internal proposal form is required for all proposals.
  • Completion of the proposal form may take several days and must be submitted before the proposal is mailed.
  • It remains in the project file on campus and does not go to the funding agency.
  • This record of project title, director, funding agency, and cost is critical for the University's overall management of grants and commitment of matching funds.
  • The signatures assure that all appropriate University officials have knowledge of the proposal and its contents, and all agree it should be submitted.
  • The proposal form is available from this site, from the "O" drive, or upon request from the Office of Grants and Contracts.
