All enrollment data will be processed by LSSU HR, shared with our benefits partner in December for auditing and packaging, and uploaded to BCBSM to be effective on January 1, 2026.

BCBSM Rx Prescription Formulary By Plan

> Formulary Documents

There is a formulary document for each plan. For reference, the number(s) refer to the specific BCBSM formulary document in the event this is needed in customer service conversations with your primary care provider, The Pool, or BCBSM.

The formulary documents for each plan are included below the plans listed above.

> Change in Broker

For 2026, LSSU is moving from our current Broker (Acrisure / 44 North) to the Western Michigan Health Insurance Pool (The Pool).

This change in broker also changes our prescription formulary.

We are renewing our medical coverage with BCBSM and will be using the BCBSM prescription formularies that outline the drugs that BCBSM covers.

Links to formulary documents are provided below the plan options listed above on this page.

You are encouraged to discuss your medications with your primary care provider to ensure there is no break in coverage.

Opt Out - Medical / Dental / Vision

> Don’t Need Medical/Dental/Vision Coverage?

This year, qualifying employees can opt out of their medical/dental/vision coverage and receive a $100/pay-period stipend ($2,600/year).
Qualifying employees are those individuals who are not covered under any LSSU medical/dental/vision plan, either as primary or as a dependent.
Employees who elect to opt out must confirm that they do not have coverage through another LSSU policyholder and acknowledge that they are declining university-provided medical/dental/vision benefits for the 2026 plan year.
