Please fill out this form to request an appointment in Testing Services. You will receive a response to this request from a Testing Services staff member to confirm or reschedule your appointment. If you have any questions, please call 906-635-2027 or email [email protected].
NOTE: All appointment requests must be submitted at least two business days prior to test date. Appointment requests that do not adhere to this stipulation may be rescheduled.
We will do our best to accommodate your exam time and will let you know if we cannot schedule this exam at the requested time and date.
*PLEASE NOTE THERE IS A $20 PROCTORING FEE DUE AT THE TIME OF THE TEST paid by cash or check (checks should be made out to "Lake Superior State University".)
You must complete the BYU registration BEFORE proceeding with the LSSU registration.
If you have not done so, you can visit the BYU FLATS website to begin the registration process.
Most exams will be given via an online format however, there are a few exams that are paper only.
You will need to call BYU to order written exams (801) 422-3512. Allow AT LEAST 2 weeks for BYU to mail your paper exams to the University Testing Center.

The TEAS exam costs $75 and can be paid by cash or check at the time of the appointment. You MUST register for an ATI student account prior to your exam.

Testing Services will email you a confirmation to the email you provided if space is available. By submitting, you also agree to Testing Services' current policies and procedures.
We reserve the right to deny your request if we do not have the space or personnel available to proctor your exam or if your professor disagrees with this request.

A copy of your LSSU exam information request will automatically be sent to your professor.
